My Health My Way - new NHS courses for those living with health conditions

My Health My Way is the name of a series of new NHS courses to support people living with health conditions.

The new programme is:

• Provided free on the NHS. • Aims to help people living with health conditions and carers overcome some of the challenges they might be facing, working with them to develop their confidence to manage. • Provides non-clinical support from specially trained health coaches who work with people on the areas where they want help. • Flexible to people’s needs and works with people in whatever way suits them best, from one-to-one coaching, telephone support, group work, online tools to structured support groups.

• Available for anyone living with a health condition or a carer over the age of 18 living in Dorset, Poole or Bournemouth (or registered with a GP here). • Accessible by referral from any health or social care professional or through self-referral by contacting 0303 303 0153 or emailing info@myhealthdorset.org.uk

Commissioned by the Dorset Clinical Commissioning Group My Health My Way is provided by My Health Dorset, a partnership between Help & Care (Dorset based charity), Dorset Healthcare University NHS Foundation Trust, Dorset County Hospital NHS Foundation Trust, Poole Hospitals NHS Foundation Trust, Royal Bournemouth and Christchurch Hospitals NHS Foundation Trust, Boots, Know Your Own Health and BH Live.

The group courses are designed to help you develop your confidence knowledge and skills to deal with the day to day problems of living with a long term health condition. It is all free and run by specially trained health coaches who can work with you on the areas where you need help. Issues covered include dealing with fatigue, low mood, pain, making changes to diets, communication and support and much more.

The next dates for a course in Dorchester are every Wednesday from 14th May at 1.30 – 4.30pm for six consecutive weeks until Wednesday 18th June (tea & coffee provided) at the Dorset Youth Association on Lubekka Way, Dorchester, DT1 1QL.

The course running in Blandford is every Tuesday from 27th May 1pm – 4pm for six consecutive weeks until Tuesday 1st July at the Blandford Forum Parish Centre, 4-9 The Plocks, Blandford Forum, DT11 7AA.
